These forms at Scouting.org are required for ALL patrol, crew or troop outings, including campouts, overnighters, hikes, service projects, summer camp and high adventure, are required for ALL scouts and scouters.

If any medications need to be taken during an outing, INCLUDING over-the-counter drugs, a Medication form must be given to the adult leader before leaving on all outings.
